This before-and-after result shows an awake lower blepharoplasty (lower eyelid surgery) combined with an awake midface lift, performed entirely under local anesthesia without general anesthesia or IV/twilight sedation.
The lower blepharoplasty addressed puffy under-eye bags through fat repositioning, creating a smooth, natural lid-cheek junction. The procedure was performed through a transconjunctival incision made inside the lower eyelid, leaving no visible external scar.
Through the same incision, an awake midface lift corrected midface descent, an age-related change in which the cheeks and midface gradually descend, creating a flatter facial contour. The procedure repositioned the descended fat pads and soft tissues to a higher, more youthful position while restoring youthful volume to the midface.
Lower blepharoplasty corrects puffy under-eye bags, while a midface lift restores descended cheek tissues. Combining the two rejuvenates both the lower eyelids and midface, creating a smoother transition between the eyelid and cheek and restoring youthful facial balance and harmony.
Both procedures were performed awake under local anesthesia only, without general anesthesia or IV/twilight/conscious sedation. Because patients remain awake, they can be instructed to gently open and close their eyes and move their mouth during surgery, allowing for real-time refinements based on a more precise evaluation of the face at rest and during natural movement. Patients often experience a faster, smoother recovery following awake plastic surgery.
