How to perform an incision technique (open) double eyelid surgery without the negative stigmata of a scar

Typical incision techniques leave a visible scar.  The visible scar occurs because the fold is made by connecting the skin to the eyelid muscle.  These surgically made folds do not move with eye opening and closing.  With eyes closed, you can see the visible scar.  And the eyelid between the fold and the eyelashes look puffy because the resultant fold is simply a non-mobile depression.  To prevent these negative effects, a dynamic fold needs to be made.  A dynamic double eyelid fold is a fold that moves with eye opening and closing.  With eyes closed, the fold is not depressed but is flat, just like the surrounding skin.  All you see is a subtle line, just like a natural double eyelid fold line.  With eye opening, the fold gradually occurs, with maximal fold occurring with eyes fully open.  Again with eyes closed, the fold progressively get thinner with no fold showing with eyes closed.  And with eyes closed, there is no visible, depressed scar.  The fullness between the double eyelid fold and eyelashes does not occur because the lymphatics (natural drainage system of tissue fluid) is not blocked by the fold being anchored down to the deeper structures.  In addition, the postoperative swelling is significantly minimized (quick recovery) because natural movement of the eyelid is not only restored but improved.  These positive effects are created by mobilizing tissue within the eyelid that is present in a natural double eyelid.  And then, this connective tissue is re-connected to the eyelid skin and the muscle that elevates the eyelid, just like a natural double eyelid.  Because the internal connection is the same as the natural double eyelid, the outcome looks like a natural double eyelid fold.  People looking at this surgically created dynamic double eyelid fold would have a hard time distinguishing between a natural vs a surgically created fold.  Often, patients who received dynamic double eyelid folds hear from others that they wish they had “natural double eyelids” like them.  

In addition, because the fold mechanism is physiologic in that the fold connection is what the natural fold was supposed to be, the eyes feel more comfortable after the surgery.   Suture technique double eyelid surgery or typical incision (open) double eyelid surgery does not make one’s eyelid feel more comfortable.  Yet, patients who receive dynamic double eyelid surgery feel more comfortable and lighter around their eyes.
